DSRIP (Delivery System Reform Incentive Payment)

 

Our goal: To promote tobacco use cessation, especially among low socioeconomic status populations and those with poor mental health, and to reduce avoidable hospital visits by 25 percent by the year 2019

With DSRIP New York State will be implementing a Medicaid Redesign Team (MRT) to restructure the health care delivery system by reinvesting in the Medicaid Program
DSRIP was passed in 2014 by Governor Cuomo, eight states have DSRIP; California, Kansas, Massachusetts, New Jersey, New Mexico, New York, Oregon, and Texas

St. Peter’s Health Partners Community Health Programs receives funding through the Alliance for Better Health Care http://allianceforbetterhealthcare.com/tobacco-cessation/ Performing Provider System (PPS) for DSRIP Project 4bi. This project is aimed at promoting tobacco use cessation.  The Community Health Programs staff works with health sites, mental health sites, and community based organizations in Albany, Schenectady, and Rensselaer counties.
